This assessment, which is completed by an Occupational Therapist  analyses your ability to complete everyday tasks such as your ability to prepare meals, access the community, complete your laundry, shower and walk, just to name a few. 

The Occupational therapist will then assist you to determine what your goals are and how these may be achieved. This may include having support workers take you shopping or accessing exercise physiology or psychology.  Recommendations may also include AT equipment for example an electric scooter so that you can access the shops.
